When casting columns, make sure the iron rods are properly placed, aligned and kept in the middle of the mould.
Simply put, let the concrete fully cover the iron rods.

You can achieve this by choosing the right size of iron rods and mould.
Avoid all instances where portions of the iron rods have to be exposed.

If not, the exposed rods will rust and lose their tensile properties, and ultimately fail to function as intended (evident in frame 2).
The ripple effect is not far fetched… you can guess.

What do I do if such mistake already exists?
Mix some rich mortar and do rough plastering over the exposed area.
