Documents to be submitted in 5 sets (Each set fastened in a folder)

A. LEGAL DOCUMENTS
1. Transfer Cert. of Title (Original TCT or Certified True Copy)
2. Real Property Tax Clearance (Latest)
3. Tax Declaration of Property
4. If lot is not in the name of applicant;
Deed of Absolute Sale
Award Notice and/or Lease Contract Agreement
Deed of Assignment or Donation
Others
5. For Corporation
Corporate Secretary’s Certification Authorizing the Signatory
B. CLEARANCES
1. Barangay Clearance (original copy or stamped “ORIGINAL RECEIVED”)
2. HOA CLEARANCE (for Building Permit)
3. Locational Clearance
4. Fire Safety Certificate
For renovation, addition, expansion, or extension
5. Bldg. Permit (if permit has no barcode, present original copy)
6. Certificate of Occupancy
7. Previously approved Bldg. Plans (especially if addition or extension)
Supplemental Documents (if applicable)
8. DPWH (if along national road)
9. DENR Environmental Clearance Certificate (ECC)
10. DOH-BHDT (for telecom/cell site projects)
11. DOLE Clearance (construction safety & health program)
12. Others
C. TECHNICAL DOCUMENTS (Application must be “duly accomplished, signed & sealed” by the Professionals and signed by the owner/s)

II. Building Plan at Scale 1:100 (5 sets signed by the owner, signed and sealed by the Licensed Engineer
1. Lot Plan signed and sealed by a Geodetic Engineer
2. Architectural Plan signed and sealed by an Architect/Civil Engineer
3. Structural Plan signed and sealed by Professional Civil Engineer/Registered Civil Engineer
4. Sanitary/Plumbing signed and sealed by Sanitary Engineer/Master Plumber
5. Electrical signed and sealed by Electric Engineer/Master Electrician
6. Mechanical signed and sealed by Mechanical Engineer
7. Fire Protection Plans if industrial/Commercial
8. CCTV Plans signed and sealed by an Electronic Engineer if Industrial/Commercial
Supporting Documents (5 sets signed and sealed by the professionals)
1. Geodetic Report/Soil Boring Test Report w/ Photocopy of Valid PRC ID & current PTR (for 3 storey & above)
2. Project Specification
3. Cost Estimate/Bill of Materials
4. Structural Analysis and Design Computation (for 2-storey above)
5. Short circuit calculation (For 30 amp. & above total connected load)
