We are so happy to introduce you to Heidi Bradshaw, our ECM Program Manager. Heidi joined the Kindred Hearts family in 2025 to lead our Enhanced Care Management (ECM) program—a service designed to provide extra coordination and support for those navigating complex health and social needs.
Heidi brings years of experience and a genuine heart to her work, making it easy for people to connect and feel understood. With a steadfast commitment to our Shasta County community, she offers a supportive presence and expert guidance to partners at every turn. To Heidi, this work isn’t just about a program—it’s about the people.
Heidi has a way of making the most complex challenges feel manageable, creating a space where both her colleagues and her clients feel truly seen. Whether she’s solving a problem or offering a kind word, her genuine heart is the foundation of everything the ECM program has become.
