✅ WHO QUALIFIES?
New Jersey resident
Age 65+ or disabled
Enrolled in NJ FamilyCare Medicaid or PACE
Living at home (not in a facility)

🛠 WHAT’S COVERED?
Bathtub removed and replaced with walk-in shower or low-threshold pan
Installation of grab bars, non-slip flooring, shower seats, and handheld sprayers
Door widening, ramps, and other accessibility improvements

📋 HOW TO START:
1. Call your Medicaid case manager (from your MCO plan) and request a Home Accessibility Assessment.
2. An Occupational Therapist (OT) will visit your home and recommend what’s needed.
3. If approved, your MCO will authorize a licensed contractor (like us) to provide an estimate and perform the work.
